Our Health Canada Medical Device License was cancelled (because we failed to renew), how do we get it back?

Cancellation of a medical device license by Health Canada requires a new filing.  This means that you will need to resubmit the documentation – as if doing so for the first time.  You will also have to submit all fees related to screening and evaluation.
